ADMISSIONS
1. INITIAL APPLICATION Complete the FAC application and return it with the non-refundable $100 application fee.
2. INTERVIEW AND TESTING Upon receipt of application and fee, you will be contacted at a later date to schedule interview and testing times.
3. COMPLETE FORMS Complete and return all applicable forms to school office:
Copy of birth certificate
Copy of social security card
Custody papers
Academic records
Immunization record

FACTS contract and voided check
Tuition and fees agreement
Field trip release form
Current photo

4. ORIENTATION Attend Mandatory Parent Orientation to get better acquainted with the school procedures and staff.
